After you have scheduled and appointment you will be sent a link to our patient portal to your email given to our office.  If you need help with registering on the portal please follow these instructions. 

How to register for patient portal. 

1) Open email sent by your providers office. Click Authenticate

2) On the next screen enter your email address, last name and date of birth. Click authenticate again.

3) Go back to your email inbox. There will be another email containing a pin number. (this expires in 24 hours-contact office if you need another pin number) Take note of this number or highlight it to copy into next section. Within this email click “Register’ to continue. 

There are three steps to complete the registration process.

1. Pin verification screen-enter first and last name and date of birth and pin number-click “Next”
2. Registration verification-enter email, phone and create a username and password-click “register”
3. Provider Authorization-Enter full name as well as todays date. If you are a parent, legal guardian or other, enter you name in the representative section and select if you are parent, guardian or other. If you are registering for yourself, leave that section blank.-Click “accept” You will now be automatically redirected to your patient portal dashboard.  .
